Revision surgery calls for greater care than the first procedure

Revision Rhinoplasty That Starts by Reassessing the Cause

Revision rhinoplasty corrects concerns from previous nasal surgery, such as contracture, deviation, implant visibility, or deformity. Because revision has more variables than a 1st operation, SUGAR Plastic Surgery uses 3D-CT to assess the internal nasal tissues in detail before planning reconstruction with an approach suited to the situation, such as autologous rib cartilage. Bringing information from your previous surgery to the consultation helps us create a more accurate plan.

Revision

What Is Revision Rhinoplasty?

Revision rhinoplasty corrects concerns from previous nasal surgery, such as contracture, deviation, implant visibility, or deformity. After a detailed assessment of the internal nasal condition, reconstruction is planned using an approach suited to the situation, such as autologous rib cartilage. Because revision has more variables than a 1st operation, a thorough consultation is essential.

The timing and technique available for revision depend on the condition of the internal nasal tissues. Bringing information from your previous surgery to the consultation helps us create a more accurate plan.

Who Needs It

You may wish to consider this procedure if you have these concerns

01When contracture has shortened or deviated the noseThe nose is reconstructed using a strong support material such as autologous rib cartilage.
02When the implant is visible or movesDuring your consultation, we discuss the approach best suited to your nose, including implant replacement or removal.
03If you are dissatisfied with the result of a previous surgeryWe identify the specific concerns and review the achievable scope of improvement with you.
04If you have experienced inflammation or other complicationsAfter assessing the tissues, we plan a safe time and appropriate technique.

Causes / Treatment

Causes and Treatments for Revision Rhinoplasty

Common reasons revision rhinoplasty is needed and correction methods for each situation.

Reason for Revision Rhinoplasty — Dropped Nasal Tip01 · When the Nasal Tip Has DroppedIf weak tip cartilage and a short columella provide inadequate support and the cartilage has descended over time, the tip is elevated by extending the septum itself or reinforcing the columella.
Reason for Revision Rhinoplasty — Nose Shortened by Contracture02 · When the Nose Has Shortened Due to ContractureContracture causes the nose to become upturned and firm while the tip shortens. Correction involves removing the implant and capsule, releasing the skin and nasal cartilage, then placing an appropriate autologous material or implant according to the condition.
Reason for Revision Rhinoplasty — Blunt Nasal Tip03 · When the Nasal Tip Looks BluntIf the tip cartilage was not narrowed adequately or the subcutaneous fat was insufficiently treated, leaving a blunt cartilage contour, correction may involve bringing the cartilage together again or grafting tip cartilage in a more appropriate shape and size.
Reason for Revision Rhinoplasty — Bridge Is Too High or Too Low04 · When the Bridge Feels Too High or Too LowIf the bridge implant is not the appropriate height, it is reshaped and reinserted while considering the tip and overall facial harmony.
Reason for Revision Rhinoplasty — Nose Is Too Wide or Too Narrow05 · When the Nose Feels Too Wide or Too NarrowIf the bridge line descending from the inner brows and orbital bones is too wide or too narrow, the pocket previously occupied by the implant is carefully released and a new graft is placed to correct the width.
Reason for Revision Rhinoplasty — Crooked-Looking Bridge06 · When the Nasal Bridge Looks CrookedIf the undersurface of a silicone implant does not match the shape of the nasal bones, it can shift to one side. Correction involves carefully reshaping the surface that contacts the nasal bones and placing it so it does not drift laterally.
Reason for Revision Rhinoplasty — Visible Implant07 · When the Implant Is Visible Through the SkinIf an implant from a previous surgery is visible through the skin of the bridge or tip, it may be replaced with a new, thinner implant or wrapped in fascia or an acellular dermal matrix before reinsertion.
Reason for Revision Rhinoplasty — Implant Seems to Move08 · When the Implant Seems to MoveEven if the implant does not actually move, persistent awareness or discomfort may be corrected by placing the implant beneath the periosteum to match the bridge contour or by changing the implant.

FAQ

Frequently Asked Questions

A.

Revision surgery is generally possible from 6 months after the previous operation. Waiting until the tissues and cartilage have stabilized allows for a more accurate assessment and a more predictable result. The appropriate timing varies with the condition of your nose and is planned after examination.

A.

Yes. A nose that has shortened and become upturned due to contracture is reconstructed by rebuilding its supporting framework with a strong material such as autologous rib cartilage. We use 3D-CT to assess the degree of contracture and the condition of the remaining tissue before creating the plan.

A.

Yes. However, we must also consider how the shape of your nose may change after implant removal. Using 3D-CT and a detailed assessment, we will recommend the safer approach—removal alone or removal with reconstruction.

A.

Existing scars and adhesions can affect tissue dissection and blood flow, which may alter the degree of swelling, recovery time, and the extent of reconstruction that is possible.

A.

We need to know the previous incision technique, implants, cartilage or injectable materials, and any history of infection or contracture to identify the cause of the current deformity and plan the extent of removal or reconstruction.

A.

Contact the clinic immediately if the implant becomes visible or exposed, the color of the nasal skin changes, or pain or pus develops. Severe breathing difficulty requires emergency medical care.
