Consent to the Collection and Use of Personal Information

SUGAR Plastic Surgery (hereinafter the “Clinic”) values your personal information and complies with the Personal Information Protection Act. This Privacy Policy explains how the personal information you provide is used, the purposes for which it is used, and the measures taken to protect it. If the Clinic revises this Privacy Policy, it will announce the change through a website notice (or an individual notice). ◯ Effective date: 2022-1-1. ❑ Personal information collected The Clinic collects the following personal information for membership registration, consultations, service applications, and related purposes. - Items collected: name, date of birth, gender, login ID, password, mobile phone number, email address, occupation, work or home telephone number, anniversary, resident registration number, and other information. ❑ Purposes of collecting and using personal information The Clinic uses collected personal information for the following purposes. - Performance of contracts for service provision: providing content. - Membership management: identity verification and identification for membership services; preventing fraudulent use by delinquent members and unauthorized use; confirming intent to register; age verification; confirming consent from a legal representative when collecting personal information from a child under age 14; handling complaints and other inquiries; and delivering notices. - Marketing and promotion: delivering promotional information from the Clinic, analyzing access frequency, and compiling statistics on members' use of services. ❑ Retention and use period The Clinic destroys personal information without delay once the purpose of its collection and use has been fulfilled, without exception. ❑ Destruction procedure and method In principle, the Clinic destroys personal information without delay once the purpose of its collection and use has been fulfilled. The procedure and method are as follows. - Procedure: Information entered for membership registration is automatically discarded when the member withdraws and, unless required by law, is not used for any purpose other than that for which it was retained. - Method: permanent deletion from the database. ❑ Provision of personal information In principle, the Clinic does not provide users' personal information to outside parties. Exceptions apply in the following cases: - when users have given prior consent; or - when required by law or requested by an investigative agency in accordance with procedures and methods prescribed by law for investigative purposes. ❑ Outsourcing of collected personal information The Clinic does not entrust your information to an outside company without your consent. If such a need arises in the future, the Clinic will notify you of the recipient and the outsourced work and obtain prior consent when required. ❑ Rights of users and legal representatives Users and their legal representatives may view or correct their own registered personal information or that of a child under age 14 at any time and may also request withdrawal from membership. To view or correct your information or that of a child under age 14, select “Change Personal Information” (or “Edit Member Information”); to withdraw membership (withdraw consent), select “Withdraw Membership,” complete the identity-verification process, and then view, correct, or withdraw directly. You may also contact the personal information manager in writing, by telephone, or by email, and the Clinic will act without delay. If you request correction of inaccurate personal information, the Clinic will not use or provide that information until the correction is complete. Personal information terminated or deleted at the request of a user or legal representative is processed as stated in the Clinic's retention and use period and protected against access or use for any other purpose. ❑ Privacy inquiries To protect customers' personal information and handle privacy complaints, the Clinic designates its director or a separate personal information manager as described below.
